Sharity Ministries Program Disclosures

OVERVIEW

Sharity Ministries, Inc., (Sharity) offers a voluntary program called a Health Care Sharing Ministry (HCSM). A HCSM is a group of individuals (members) who share a common set of ethical or religious beliefs and voluntarily choose to share in the payment of their medical expenses in accordance with those beliefs, without regard to the state in which a member resides or is employed. Membership cannot be transferred to anyone other than the member and his/her eligible enrolled dependents.

Services are offered on a faith-based tradition of mutual aid, neighborly assistance, and burden sharing. Sharity is specifically tailored for individuals who believe in helping others. As participants in an HCSM, Sharity members agree not to subsidize self-destructive behaviors or lifestyles. Sharity is NOT insurance and provides no guarantee to pay.

All members of the HCSM program offered by Sharity are required to declare their acknowledgment of the Statement of Beliefs and to attest that they are of like mind with those beliefs. Sharity’s Board of Directors is the final authority of the interpretation of the Bible and its application to Sharity Ministries.

STATEMENT OF BELIEFS

  1. We believe the Bible alone is the inspired Word of God: therefore, it is the final and only source of absolute spiritual authority.
  2. We believe in the triune God of the Bible. He is one God who is revealed in three distinct Persons – God, the Father; God, the Son; and God, the Holy Spirit.
  3. We believe Jesus Christ was God in the flesh – fully God and fully man. He was born of a virgin, lived a sinless life, died on the cross to pay the penalty for our sins, was bodily resurrected on the third day, and now is seated in the heavens at the right hand of God, the Father.
  4. We believe that all people are born with a sinful nature and can be saved from eternal death only by grace alone, through faith alone, trusting only in Christ’s atoning death and resurrection to save us from our sins and give us eternal life.
  5. We believe in the bodily resurrection of all who have put their faith in Jesus Christ.
  6. All we believe and do is for the glory of God alone.

VOLUNTARY PARTICIPATION

Members are voluntary participants of an HCSM program. Membership and participation in Sharity’s HCSM program, such as the sharing of monetary contributions, is voluntary. Enrollment is NOT a contract. Members are free to withdraw participation at any time. Sharity collects a requested “monthly contribution” from members to facilitate the sharing of eligible medical expenses.

GUIDELINES

Sharity facilitates sharing by establishing the guidelines that generally define the sharing of eligible expenses between members of Sharity’s HCSM programs (“Guidelines”), and more specifically defines the sharing of eligible expenses between members of each Sharity program outlined in the individual member guide(s) available on Sharity’s website and provided at the time of enrollment. The Guidelines and Sharity member guides are not contracts and do not constitute an agreement, a promise to pay, or an obligation to share.

The Guidelines are intended to ensure that every member has paid his/her own medical expenses as they are financially able before requesting others to share in the cost of remaining eligible medical expenses. The Guidelines generally define when a member is eligible for sharing requests, while individual member guide(s) detail what type of expenses may be eligible for sharing per program, including specific limitations, exclusions and requirements for sharing eligibility, so all members can expect a reasonable and equitable level of sharing. The total amount for which sharing is requested and shared each month is published monthly in a newsletter to members that is available on Sharity’s website./p>

Sharity programs may exclude or have sharing limitations for pre-existing conditions. Members are required to fully disclose pre-existing conditions as part of their enrollment in Sharity programs. Sharity reserves the right, on behalf of members, to exclude sharing eligibility for any pre-existing conditions, whether disclosed at the time of enrollment or discovered after the effective date of membership. Furthermore, a member’s medical sharing request(s) is not eligible for sharing under these two scenarios:

  1. When a member receives care within the first 60 days of their active date and cancels membership within 30 days of receiving medical care, the request will not be eligible for sharing. For example, if the member receives medical care on the 59th day of membership and cancels prior to the 89th day, the medical expenditure would not be eligible for sharing. The 30-day cancellation stipulation does not apply during the last 90 days of your program year.
  2. When a member receives or requires surgery within the first 60 days of becoming a member, except in the case of an accident, the request will not be eligible for sharing. For example, planned surgeries are not eligible for sharing within the first 60 days of the member’s active date; however, if you were in an automobile accident during this period, this medical sharing request would be eligible for sharing.

Sharity reserves the right to make updates to the Guidelines and member guides at any time on behalf of its HCSM program members. The Guidelines and member guides in effect at the time of service will supersede all previous versions of the Guidelines and member guides. Members will be notified of updates.

SHARING REQUESTS AND USE OF FUNDS

After receiving an eligible sharing request from a member or a provider, Sharity will assign the eligible expense(s) for sharing, less the amount of personal responsibility required, called the Member Shared Responsibility Amount (MSRA).

Voluntary “monthly contributions” are received from each member, each month. Member contributions may be applied towards administration of Sharity programs, or general overhead costs. This does not include distribution compensation. Administrative costs are subject to change by Sharity.

HCSM TAX MATTERS

Members should always consult with a tax professional to determine whether participation will have tax implications.

Dispute Resolution & Arbitration

Sharity’s HCSM programs are a voluntary association of like-minded people who come together to assist each other by sharing medical expenses without establishing legal obligations. However, it is recognized that differences of opinion may occur, and that a methodology for resolving disputes must be available. Therefore, by becoming a member of Sharity’s HCSM program, you agree to resolve any and all disputes you have with Sharity using the steps set forth in the applicable member guide, which includes resolving any dispute via legally binding arbitration.

AUTHORIZATIONS & ACKNOWLEDGMENTS

  • I authorize Sharity Ministries to collect the monthly contribution amount as a recurring monthly transaction.

  • I authorize the monthly contribution amount to be processed immediately upon completion of my application.

  • I understand that the application fee will be refunded automatically if all individuals on my application form fail to attest to the Sharity Statement of Beliefs or if I withdraw my application prior to my membership effective date.

  • I understand that the application fee will not be refunded if, in the course of applying, I fail to respond to written or verbal inquiries from Sharity for more than sixty days.

  • I understand that I have requested voluntary participation in a Sharity HCSM program.

  • I affirm that the name and personal information provided on this form are true and correct.

  • I understand Sharity has the authorization to contact providers to request the release of medical records on behalf of the member.

  • I understand that Sharity Ministries, Inc., and its affiliates have the authority to request my medical records to facilitate the sharing of medical expenses.

  • I understand that Sharity is not an insurance company, and that the HSCM program is not an insurance product or policy.

  • I affirm that I understand and accept the disclosures presented above.

Legal Notices

The following legal notices are required by state law and are intended to notify individuals that health care sharing ministry programs are not insurance and that Sharity Ministries does not provide any guarantee or promise of payment for any medical costs or expenses incurred by members of the HCSM.

PARTICIPATION IN THE RELIGIOUS ORGANIZATION OR A SUBSCRIPTION TO ANY OF ITS DOCUMENTS SHALL NOT BE CONSIDERED INSURANCE. ANY ASSISTANCE YOU RECEIVE WITH YOUR MEDICAL BILLS WILL BE TOTALLY VOLUNTARY. NEITHER THE ORGANIZATION OR ANY PARTICIPANT SHALL BE COMPELLED BY LAW TO CONTRIBUTE TOWARD YOUR MEDICAL BILLS. Notice: The ministry facilitating the sharing of medical expenses is not an insurance company. Neither the guidelines nor the plan of operation of the ministry constitutes an insurance policy. Financial assistance for the payment of medical expenses is strictly voluntary. Participation in the ministry or a subscription to any publication issued by the ministry shall not be considered as enrollment in any health insurance plan or as a waiver of your responsibility to pay your medical expenses.

Maine Statute title 24-A, §704

Notice: The eligible entity that operates this health care sharing ministry, Sharity Ministries, Inc., is not an insurance company and the financial assistance provided through the ministry is not insurance and is not provided through an insurance company. You are personally responsible for payment of your medical bills regardless of any financial sharing you may receive from the organization for medical expenses. You are also responsible for payment of your medical bills if the organization ceases to exist or ceases to facilitate the sharing of medical expenses.
